Your name, age, current address

Evidence of your age and residence

Evidence about your disability

Relevant medical reports

NDIS can ask you for more information. After you provide the required information , NDIS will send you a letter of their decision about your eligibility for NDIS.

2.NDIS planning booklet helps you to provide information in regard to plan a meeting with NDIS. When your plan get approved, you will receive your plan either in the mail or in person and trough the MyPlace Portal. During your planning meeting you will be asked about your goals /activities/tasks you want to achieve.It could be a goal to use public transport independently , assistance with household activities or social inclusion.
